Context: Health care practitioners named to lead or oversee the sub-districts are sub-district leaders/managers. A series of survey and monitoring studies have revealed poor sub-district leadership and management that can undermine sub-district health systems and contribute to the overall effects on resilient health systems. Via the community-based health planning and services (CHPS) definition in Ghana, the Sub-district health system is the first primary health care delivery point aimed at taking health care to customers’ door steps. The aims of the study were to: define the leadership styles displayed by sub-district leaders, examine the correlation between sub-district leadership/management styles and the performance of their subordinates, and analyse the relationship between sub-district leadership styles and performance. Methods: A cross-sectional nature of the survey was employed. Based on the Ghana Health Service performance assessment criterion, the average performance of the leaders was determined. Principal component and exploratory factor analyses were used to determine the demonstrated leadership styles, while correlation and regression analyses were used to determine the relationship between leadership styles and subordinate performance, leadership styles and performance of leaders. Results: The results revealed that the leadership styles displayed by the leaders are transformative and supportive and both are positively linked to their subordinates’ success. However, the style of transformational leadership appears to contribute more to the success of the subordinates than the leaders of the supportive style. Both leadership styles are each significantly linked to the leaders’ results. Additionally, the performance of a greater overall leader is correlated with the performance of a higher subordinate. A higher rating of leadership skill is often correlated with higher performance by leaders. Conclusion: The leadership styles of sub-district leaders or administrators have an immense impact on the success of their subordinates and the leaders themselves. There was a very positive linear correlation between leadership ability and leadership efficiency. The connection between the performance of leaders and the performance of subordinates, leadership ability, and the performance of leaders was positively linked. The types of transformational and constructive leadership are positively linked to the subordinates’ success. In order to obtain in-depth data on the management quality styles of sub-district leaders, qualitative research is needed.
